Medicaid Waiver for Nursing Home Transition and Diversion

 

The Home and Community-Based Services Nursing Home Transition and Diversion (NHTD) Medicaid waiver provides supports and services to people with disabilities and the elderly who wish to continue living in their homes. Under the NHTD waiver, AIM provides service coordination, Community Integration Counseling, Positive Behavioral Inteventions and Supports and independent-living-skills training.

Eligibility

  • Be capable of living in the community with assistance from available informal supports, non-Medicaid supports and/or Medicaid State Plan services and be in need of one or more waiver services

  • Be eligible for nursing-home level of care

  • Be authorized to receive Medicaid Community Based Long Term Care

  • Be at least 18 years of age or older

  • Be considered part of an aggregate group that can be cared for at less cost in the community than a similar group in a nursing home

  • Choose to live in the community as a participant in this waiver rather than in a nursing home, and not participate in another HCBS waiver


Medicaid Waiver for Traumatic Brain Injury

 

The Home and Community-Based Services TBI Medicaid waiver provides supports and services to people who have survived a traumatic brain injury and want to continue living in their homes. Under the TBI waiver, AIM provides service coordination, Community Integration Counseling, Positive Behavioral Intervention and Supports and independent-living-skills training.

Eligibility

  • Have a diagnosis of TBI or a related diagnosis

  • Be eligible for nursing-home level of care as determined by a Patient Review Instrument (PRI) and SCREEN

  • Be a Medicaid recipient

  • Be 18-64 years old

  • Choose to live in the community rather than a nursing facility

  • Have or find a living arrangement that meets the individual’s needs

  • Be able to be served with the funds and services available under the HCBS/TBI waiver and New York State Medicaid State Plan

  • Not participate in another HCBS waiver.
