Diabetes Self-Management Program

medical.clipboard.pngThe Diabetes Self-Management Program (DSMP) is a six-session, peer-led health education program for people living with diabetes, and their caregivers.

The program is designed to complement the health care that participants may already be receiving. The purpose is to enhance one’s skills and ability to manage his/her health and maintain an active and fulfilling lifestyle.

The workshop sessions will be facilitated by DSMP-trained staff and will take place in Allegany, Chemung, Steuben and Schuyler counties.

DSMP is funded by the New York State Department of Health Balancing Incentive Program. This program is designed to help lower Medicaid costs in New York state by transforming and improving our long-term care system.
